Masson’s HemangiomaThe patient is a 51-year-old woman with a history of left knee synovial sarcoma, initially diagnosed at age 9. The sarcoma recurred at age 13 and at age 26 she had metastases to the left lower lobe lung. She has been disease free since. Screening mammogram now detects a new nodule in the left upper inner quadrant. Overall, it is well circumscribed but has a few worrisome features. Core biopsies show a well demarcated lesion which consists of anastomosing vascular channels separated into lobules by wide fibrous septae. The endothelial cells are enlarged and hyperchromatic. Scattered mitotic figures are seen. Conservative excision for complete evaluation is recommended. The excised specimen confirms the diagnosis of intravascular papillary endothelial hyperplasia (Masson’s hemangioma).
