CASE OF THE MONTH

Blastomycosis

A 48 year-old-gentleman presents with a right middle lobe lung mass. He has no risk factors for malignancy, including smoking. The resected specimen shows a rubbery, well-circumscribed, white lesion measuring 8 cm maximally. Sections demonstrate a fibrocaseous mass with central fibrosis and necrosis. The tissue surrounding the mass consists of benign lung tissue with scattered lymphoid follicles and loose granulomas. Scattered fungal elements are seen on H&E, PAS, and GMS stains. These elements are primarily yeast forms, some of which show broad based budding. Serological studies for Blastomycosis are confirmatory.
