CASE OF THE MONTH

Signet Ring Adenocarcinoma

The patient is a 68 year old female who underwent lung wedge biopsy for a right lung mass. On gross examination there was a single well circumscribed lung nodule. Histologic sections demonstrate nests and sheets of signet ring cells.

Immunoperoxidase stains show that the tumor cells stain positive with CK-7, Napsin A, TTF-1, EMA and negative with CDX-2 and CK-20. Mucin stain is positive.

The staining pattern is compatible with an adenocarcinoma of lung origin. Signet ring cell adenocarcinoma is an uncommon variant of lung adenocarcinoma.
